The grade is based on over 30 national performance measures that assess errors, accidents, injuries, and infections, as well as the effectiveness of the hospital's systems in preventing harm.

The Leapfrog Hospital Safety Grade is a unique rating program focused on hospital prevention of medical errors, infections, and injuries that result in over 500 patient deaths daily across the nation. The grades are updated semiannually in the fall and spring, and the program is peer-reviewed, fully transparent, and accessible.
